3. Glasses if vision requires a prescription
4. Hearing aids if required
5. Last Medical Examination Report Card (MER)
6. List of medications with dosages

- Cardiologist clearance letter for other chronic conditions may be required before clearance

The physical exam will includes:
Listen to Heart and Lung
Evaluate any Muscular weakness or disease
Examine and check for abnormalities of the liver, abdomen, and nervous system
Check for spine deformities
Check pulse and blood pressure
Evaluate Past or recent illnesses and injuries
Perform Vision test, hearing test
Examine eyes, ears, mouth, throat

- Complete section 1
- Bring it with you to your appointment
2. Medical Examiner's Certificate (MEC): Print and bring it with you to your appointment.

- Skill Performance Evaluation: The SPE certification allows drivers with missing or impaired limbs to drive CMVs across state lines if they have been fitted with (and are wearing) the right prosthetic device, and the driver can demonstrate the ability to drive the truck safely.
